We invite current FVMA patients to use this secure and convenient online form to request a non-emergency appointment at either of our Fox Valley Medical Associates (FMVA) offices. After the receipt of your completed form, a FVMA representative from our Aurora or Batavia office (depending on your location request) will contact you within the next 2 business days. If you are a new patient, please contact either office directly.
