Fox Valley Medical Associates accepts most major insurance carriers. Please note that these contracts are frequently reviewed and subject to change. If you have any questions about your plans’ participation with our office, please contact your carrier’s customer service department. Below is a partial listing of plans that our office is currently accepting:

Insurance Forms

Please allow 5-7 business days for completion of any forms such as FMLA, return to work, return to school, etc. Patients are requested to complete as much of the form as possible. Some forms may require an appointment with the physician to complete. Our office will notify you when the forms are ready to be picked up.

HMO Referral Process

Fox Valley Medical Associates doctors participate in the Fox Valley Medicine IPA (Independent Physician Association) for managed care insurance plans. These plans require patients to obtain a referral for certain medical services. As the “gate keeper”, the primary care physician will coordinate the patient’s plan of care.

Referrals to a specialist, hospital, medical supply company, and laboratory or therapy facility are initiated through the primary care physician. Referrals may take anywhere from 24 hours to 7 days to complete.

Often the insurance company requires the office notes and recommendations from the specialist for review prior to authorization. Our referral coordinator will assist you in contacting the specialist for additional information. If we are unable to obtain this information, our office may contact you for assistance in obtaining the necessary information. Referrals cannot be backdated.

Referrals are mailed to the patient. It is the responsibility of the patient to make sure you have an approved referral prior to scheduling any appointments or testing. Emergent referrals are for life threatening conditions only.

When seeing one of our specialists, a referral may be required. If you do not have your referral at the time of your visit, you may be asked to reschedule your appointment or sign a waiver. If you sign a waiver, you are financially responsible for the total amount of the visit and it is due at the time of service.
