Accepted Insurance Plans
Fox Valley Medical Associates accepts most major insurance carriers. Below is a partial list of plans our office currently accepts:

Insurance Forms
It may take 5-7 business days for us to complete any forms, such as FMLA, return to work, return to school, etc. We ask patients to please complete as much of the form as possible. Keep in mind some forms may require an appointment with the physician to complete.
Once they’re completed, our office will notify you when the forms are ready to be picked up.
HMO Referral Process
Fox Valley Medical Associates doctors participate in the Fox Valley Medicine IPA (Independent Physician Association) for managed care insurance plans. These plans require patients to obtain a referral for certain medical services. The primary care physician will coordinate the patient’s plan of care when this is the case.
The primary care physician will initiate referrals to a specialist, hospital, medical supply company, laboratory, or therapy facility. Please be aware that referrals may take anywhere from 24 hours to 7 days to complete.
Often the insurance company requires the office notes and recommendations from the specialist for review before authorization. Our referral coordinator will help you contact the specialist for any additional information. If we’re unable to obtain this information ourselves, our office may contact you and ask for your help. Referrals cannot be backdated and are mailed to the patient when ready.
The patient must have the approved referral before they can schedule an appointment or testing. Emergency referrals are for life-threatening conditions only.
Seeing one of our specialists may require a referral. If you don’t have one at the time of your visit, you may be asked to reschedule your appointment or sign a waiver. If you sign a waiver, you’re financially responsible for the total amount of the visit, which is due at the time of service.
