Your First Visit to FVMA

The time you have with your doctor is very important to your health. It is a good idea to prepare and plan ahead for your visit. New patients should arrive at least 15 minutes prior to your scheduled appointment time to complete the necessary paperwork. Be prepared to present your current insurance card, photo I.D., and list of all your medications including vitamins and supplements. Make a list of the questions that you want to ask.

Before seeing the doctor, you will need to complete patient registration forms, medical history, acknowledgement of Privacy Practices, an authorization to release confidential health information and a financial agreement.

On an annual basis, we will require an update of all your information and registration forms. At every visit, our support staff will verify your address, telephone number and insurance information. Please be prepared to show your insurance care and photo I.D. Even though your insurance plan may not have changed, our staff must verify that we have the most current information on file to avoid billing issues. Please be sure to provide all insurance cards.

It is helpful to give the staff as much information as possible regarding the nature of your visit. This will ensure that the proper appointment time is given to you.

For your convenience, well-child visits, school physicals and routine annual exams should be scheduled early to avoid delays. We encourage you to contact the office as soon as possible to schedule these visits. Well-child follow-up visits should be scheduled before leaving the office. This will ensure that your child is seen at the appropriate time for immunizations.
