What To Expect During Your First Visit
Thanks for choosing us to guide you on your health journey. Here are some steps we recommend you take to help make your first visit as easy and stress-free as possible.
Tips for New Patients
- Arrive at least 15 minutes before your scheduled appointment time. You’ll need to complete the necessary paperwork before seeing the doctor. This includes filling out:
- New patient registration forms
- Your medical history
- An acknowledgment of privacy practices
- An authorization to release confidential health information
- A financial agreement.
- Be prepared to present the following:
- Your current insurance card – Even though your insurance plan may not have changed, our staff must verify that we have the most current information on file to avoid billing issues. Please be sure to provide all insurance cards.
- Photo I.D.
- A list of all your medications–including vitamins and supplements.
- Make a list of the questions that you want to ask.
- Give the staff as much information as possible regarding the nature of your visit. This helps determine your proper appointment time.
At every visit, our support staff will verify:
- Your address
- Telephone number
- Insurance information
Scheduling Tips
- For your convenience, well-child visits, school physicals, and routine annual exams should be scheduled early to avoid delays. Contact the office as soon as possible to schedule these visits.
- Well-child follow-up visits should be scheduled before leaving the office. This will ensure your child is seen at the appropriate time for immunizations.
